Every listing is pulled from the Illinois Department of Public Health (IDPH) Licensing & Certification System public Facility Lookup (Assisted Living/Shared Housing Establishment and Long Term Care Facility) for Cook, DuPage, Lake, Will, Kane and McHenry counties. Only Active licenses are shown; inactive, closed and change-of-ownership records are excluded. We refresh quarterly.
